We ask everyone who attends a termly mat class or private session to fill in a health screening and waiver form. This asks for your name, address, email address and phone numbers and has a large health information section.
To be able to do our job as Pilates teachers, we need to ask you information about health conditions and injuries. It is very important you give accurate and current information to us, and keep us up to date with your state of health.
Screening forms are stored securely as either paper copies in a locked filing cabinet or online. Only Becky Dickson has access to the filing cabinet and electronically stored copies.
We do not discuss teaching you or any details of your health with anyone outside of Pilates Pathway.
We may ask your permission to talk or email your physiotherapist, consultant, doctor, or other health professional if we think this would benefit you. You would be entitled to see this correspondence if requested.
We welcome your comments or questions about our website and have provided email links for that purpose. We will share your comments and questions with our customer service representatives and those employees most capable of addressing your questions and concerns.
